I am a new graduate mentoring Care home and want to give it more than just the usual guide documentation there. Hmmm, good question. If you are looking beyond the text book SOAP traditional note, which could strain myself. We had documentation of the sample for all new graduates and put them in a binder of reference. We also gave them a copy of our facilities accepted abbreviations … (BTW, we still can not use RW for rolling walker or vitamin A to help small). I still think there is no better way to learn what the experience ….
